CMS Provides Teaching-Physician Clarification for Evaluation & Management Services Documentation
The Centers for Medicare and Medicaid Services (CMS) recently clarified that requirements for evaluation and management documentation in the teaching setting must include notes by both the resident and the teaching physician.  Further, the guidance includes sample documentation language and vignette scenarios for the appropriate Medicare billing of teaching physician services.

Teaching physicians are still required to document whether they were physically present for the critical or key portions of the service or procedure; however, this directive does allow teaching physicians to reference resident notes in their documentation for the review of systems and/or patient history.
